PHILIPPINE ACADEMY OF OPHTHALMOLOGY 2010 AWARDS OF DISTINCTION

AWARD CATEGORIES:

  1. PAO Distinguished Service Award
  2. PAO Outstanding Researcher in Ophthalmology Award
  3. PAO Outstanding Ophthalmic Educator Award
  4. PAO Distinguished Community Service Award
  5. PAO Outstanding Leadership Award

Criteria: (nominee must comply with all criteria per category)

DISTINGUISHED SERVICE

  • Must be a member in Good Standing for at least ten (10) years
  • Should have unselfishly contributed invaluable service for the upliftment & welfare of PAO for at least ten (10) years (cumulative)
  • Should have not held an elective position in the Executive Council for more than or equal to 10 years (cumulative)
  • Should have shown unquestioned loyalty to PAO
  • Should be of high ethical and moral standards

OPHTHALMIC RESEARCH

  • Must be a member in Good Standing
  • Should have made significant contributions to ophthalmic knowledge through research in the country and/or to the world
  • Must be actively involved in Eye Research for at least twenty (20) years
  • Must have published at least 20 primarily authored Eye Research papers

OPHTHALMIC EDUCATION

  • Must be a member in Good Standing
  • Must have been at least an Associate Professor or its equivalent in academic rank
  • Must be actively involved in teaching ophthalmology for at least twenty (20) years
  • Should have made significant contributions to the improvement of ophthalmic education and training in the country

COMMUNITY SERVICE

  • Must be a member in Good Standing
  • Must be consistently & actively involved in providing Community Service for the underprivileged for at least ten (10) years or practiced ophthalmology in an under-served community for at least ten (10) years
  • Should be of high ethical and moral standards

LEADERSHIP

  • Must be a member in Good Standing
  • Must have held an elected position in the Executive Council or local chapters for at least ten (10) years (cumulative)
  • Should have initiated or established exemplary programs in the service of Philippine Ophthalmology
  • Should have significantly contributed to the growth and improvement of the PAO
  • Should have shown unwavering loyalty to PAO
  • Should be of high ethical and moral standards

General Rules and Guidelines

  1. The Committee on Awards is in-charge of the PAO Awards of Distinction.
  2. Committee on Awards is an ad hoc committee formed by the President who will appoint the Chair. The President or Chair will appoint four members to the committee.
  3. PAO Awards of Distinction are given every five years beginning in the year 2005.
  4. There are five award categories: Distinguished Service, Ophthalmic Research, Ophthalmic Education, Community Service and Leadership
  5. The awardee shall receive a medallion and a certificate/citation and any other appropriate gift approved by the Executive Council.
  6. Rules, criteria and mechanics will be recommended by the Committee on Awards to the Executive Council for approval.
  7. Nominations should be submitted to the Committee on Awards who will send their recommendations according to rank (points/scores) to the Executive Council for approval.
  8. All awardees should be approved by the Executive Council by majority (two-thirds) of those present during a regular council meeting. The manner of voting will be decided by the Executive Council.
  9. There can be more than one awardee per award category per award period.
  10. A person can only be awarded once in his lifetime under each award category.
  11. A person can receive more than one award category in the same year.
  12. Elected members of the Executive Council and members of the Committee on Awards are automatically disqualified from being nominated.
  13. Any PAO member of good standing may nominate.
  14. A person may nominate more than one person but only to one award category each.
  15. The nominee must accept the nomination by signing the nomination form.
  16. All nominations must be accompanied by a Letter of Justification and supporting documents relevant to the award category being nominated, such as curriculum vitae, certificates, journals, letters, publications, pictures, sworn affidavits, etc., for verification and authentication.
  17. All photocopied documents must be certified true copy.
  18. Nomination forms and accompanying documents must be sent to the PAO Secretariat either by mail or courier service. No fax or email submissions are allowed.
  19. The Committee will only review, scrutinize and rank nominations that have adequately complied with the requirements.
  20. All minutes, records, transactions, communications, meetings, etc., of the Committee on Awards are open to the general membership.

Reminders:

  • Please be advised that incomplete nominations will be disregarded by the Committee on Awards
  • Verbal or phoned-in nominations are not acceptable
  • Tampering or heavy erasures on the nomination form will render it invalid
  • You may photocopy the nomination form or request for more from the PAO Secretariat
  • No nominations will be accepted after the deadline of submission

Awarding of Winners:NOVEMBER 20, 2010 during the Fellowship Night of the 2010 Annual Meeting
